This Chemical Services Disclaimer and Agreement is entered into between [Taylor Correa LaGrone/Taylored Up Hair], located at 4750 New Jesup Highway, Brunswick, GA, 31525, and any client booking with [Taylor Correa LaGrone/Taylored Up Hair], effective as of July 9, 2024.

1. Services
Hairdresser agrees to perform one or more of the following chemical services for the Client:
Highlighting
Permanent color
Demi permanent color
Color removal

2. Risks and Dangers
Client acknowledges and understands that chemical services such as those listed above can involve certain risks and dangers, including but not limited to:
Potential allergic reactions or sensitivities to chemicals used.
Risk of chemical burns or irritation to the scalp, skin, or eyes.
Potential damage to hair strands due to chemical processing.
Other unforeseen risks related to chemical reactions or interactions.

3. Consultation and Disclosure
Prior to performing any chemical services, Hairdresser has provided Client with a consultation where the following was discussed:
The specific chemical services to be performed.
Potential risks and dangers associated with each service.
Precautions taken by Hairdresser to minimize risks and ensure safety.
Client's medical history and any known allergies or sensitivities.

4. Client Responsibilities
Client agrees to:
Disclose any known allergies, sensitivities, or medical conditions that may affect the safety of the chemical services.
Follow Hairdresser's instructions before, during, and after the chemical services to minimize risks.
Inform Hairdresser immediately of any discomfort, pain, or adverse reactions experienced during or after the services.

5. Limitation of Liability
Hairdresser shall not be liable for any damages, injuries, losses, or claims arising out of or related to the chemical services performed, except to the extent caused by Hairdresser's gross negligence or willful misconduct.

6. Indemnification
Client agrees to indemnify and hold Hairdresser harmless against any and all claims, liabilities, damages, losses, costs, and expenses, including reasonable attorney fees, arising out of or related to the chemical services performed, except to the extent caused by Hairdresser's gross negligence or willful misconduct.

7. Consent and Agreement
By booking, Client acknowledges that they have read and understood this Agreement, including the risks associated with the chemical services, and voluntarily consent to receive the services described herein.
